Terms of Business and Informed Consent Form

This document outlines the terms of business and informed consent for services provided by Simone Holz – Holistic Nutritional Therapist. By booking and using my services, you agree to the terms outlined below in accordance with Irish and European consumer protection laws.

Our services include Nutritional Therapy and Nutrition and Health Coaching support, as well as holistic therapies such as Inner Harmony Healing, Soul Alignment Therapy, and Quantum Balance Therapy. These services aim to promote health, peak performance, and increase your wellbeing through personalised nutrition and lifestyle programmes.

Nutritional Therapy and Nutrition and Health Coaching are recognised as complementary therapies and do not replace medical treatment. The same applies to the holistic and energetic therapies. Clients are encouraged to consult their medical professional for any serious health concerns.

Informed Consent and Client Responsibilities

By engaging with Simone Holz – Holistic Nutritional Therapist, you acknowledge and agree that:

• Nutritional Therapy and Nutrition and Health Coaching are tailored to individual needs, and results may vary.
• We cannot diagnose, treat, cure, or prevent an illness in you or even treat medical conditions and do not replace professional medical care.
• You are responsible for consulting your doctor about any health concerns and informing them of any nutritional programme or supplements recommended.
• It is essential to disclose all relevant medical history, current medications, herbal supplements, and lifestyle habits.
• If you have questions about your nutritional plan, you should contact your practitioner for clarification.
• You must inform your nutritional therapist before continuing a supplement programme beyond the initially agreed period.
• Holistic Healing Counselling/Therapy services do not replace medical, psychological, or psychiatric care.
• We are not liable for any decisions made by you during or following our service or training that may concern your life and health or the life and health of others you may know. We cannot guarantee results.
• Any material you see, hear or read is meant to be used for educational purposes. 
• Recording consultations using any electronic media is not permitted without written consent from both parties.

Booking, Payment, Cancellation and Refund Policy

Booking Requirements

• Inner Harmony Healing, Soul Alignment Therapy, and Quantum Balance Therapy must be booked at least 24 hours before the first session.
• Nourish and Flourish Coaching & Reconnect Method must be booked at least one week in advance.
• Clients booking the Nourish and Flourish Coaching or Reconnect Method programmes will be required to complete a signs and symptoms questionnaire, health history, lifestyle assessment, and food diary, to be submitted 72 hours before the session.
• For all the sessions, especially the Healing Counselling sessions, clients should be in a quiet place, a comfortable setting with water if needed, free from interruptions.

Payment Terms

• A 50% deposit is required prior to the session to confirm the booking.
• Payments for sessions must be made in advance via Bank of Ireland or Revolut to Simone Holz’s designated account.
• The remaining balance must be paid before the session begins.

Cancellation and Refund Policy

• Cancelling or rescheduling sessions is permitted up to 24 hours before the appointment with no fee.
• Full session charge is applied for all sessions cancelled within 24 hours of the appointment.
• Session fees are non-refundable after you have attended the appointment.
• If you arrive late, your session may be shortened to respect other scheduled appointments. If you arrive more than 15 minutes late, your appointment will be considered a no-show, cancelled, and fully charged. Refunds or rescheduling will not be available in this case.
• Session packages are non-refundable once booked, and you cannot be transferred to a family member or friend.
• This Cancellation and Refund Policy applies to all in-person and online appointments. Please read this policy carefully before making a booking.

Refund Policy

• You’ll be refunded within 14 business days, less any reasonable cost that has already been incurred by Simone Holz – Holistic Nutritional Therapist, such as, but not limited to, administration fee if applicable, books and manuals which have already been issued to you.

Confidentiality and Data Protection 

In accordance with the General Data Protection Regulation (GDPR) and the Irish Data Protection Act 2018, we ensure that all personal data is:

• Collected for legitimate purposes and processed lawfully.
• Stored securely and kept confidential.
• Not shared with third parties without written consent.

You have the right to access your case notes. If you wish to obtain a copy of your records, please email simoneholznutrition@gmail.com. Requests will be processed within 30 days.

By signing this document, you consent to the processing of your personal data for the purpose of your consultations.
